Certified medical examiner (CME) retraining is needed to stay on the Registry
Federal Motor Carrier Safety Administration (FMCSA) announced January 11, 2023, the requirement for the five-year refresher and ten-year recertification training and timelines.
The National Registry of Certified Medical Examiners (NRCME) was created in May 2014. Many CMEs were certified in 2013 before the rule took effect. These CMEs are coming due in 2023 for the ten-year recertification. The five-year refresher may also be due or overdue for many.
CME training requirements
Drivers of commercial motor vehicles (CMVs) operating in interstate commerce must be medically qualified by a certified medical examiner (CME) active on the Registry. A CME must meet the training requirements under 390.103 and 390.111 based on the following timeline:
- Initial: Complete initial medical examiner training and certification by a nationally accredited medical continuing education organization.
- Five-year refresher: Review the five-year refresher training course between years four and five as a CME.
- Ten-year recertification: Recertify with a nationally accredited organization between years nine and ten.
Five-year refresher
For many CMEs, the five-year point of being on the Registry passed in 2019 or earlier. In mid-2022, FMCSA made the five-year refresher training available. CMEs due or overdue for the five-year refresher in 2022 must have completed the training within their registry account by December 31, 2022. In 2023 and beyond, CMEs should complete the refresher training between years four and five, before the five-year deadline.

Ten-year recertification
The initial certification is for ten years. CMEs due for the ten-year recertification must take the course via an authorized private-sector provider.
Due to FMCSA delays making the five-year refresher course available, CMEs coming due for the ten-year recertification in 2023 have until December 31, 2023, to complete the process.
The CME must take the exam through one of only two approved testing organizations, Prometric and PSI.
Key to remember: Carriers should confirm with CMEs and provider organizations that they plan to complete the five-year refresher and ten-year recertification requirements before deadlines pass.
