New form may affect drivers with Type 2 diabetes
Commercial truck and bus drivers who have non-insulin-treated diabetes may soon need to have their healthcare providers complete a new DOT form before renewing their medical cards.
The Federal Motor Carrier Safety Administration (FMCSA) is working on final approval for a new “Non-Insulin-Treated Diabetes Mellitus Assessment Form.” This optional form will allow DOT medical examiners to obtain information from a driver’s treating physician about the “treatment, management, and control” of the driver’s diabetes. It could help the examiner make “an informed and sound physical qualification determination,” the FMCSA says.
Once approved, the form will be known as the MCSA-5872. It will be posted as a fillable PDF on the FMCSA’s website along with other medical forms.
Because it’s optional, medical examiners will have discretion to decide if they want the form to be completed. They can also decide to require completion of the form as a condition for qualifying a driver.
Be proactive
Drivers with diabetes who are not being treated with insulin may want to take the proactive step of having their healthcare provider complete the form just prior to the driver getting a new DOT medical exam. This could head off delays caused by medical examiners who need more information about a driver’s condition before making a certification decision.
Treating healthcare providers may provide the completed form directly to the driver to take to the medical examiner or may fax or scan and email the form directly to the examiner.
A similar process
The new form will be similar to one currently used by drivers who take insulin. Under 49 CFR 391.46, drivers who use insulin can be certified for up to 12 months if they first have their treating clinician complete form MCSA-5870, the Insulin-Treated Diabetes Mellitus Assessment Form, and provide it to their medical examiner.

Though drivers who take insulin cannot be certified if they don’t go through the process outlined in 391.46, there is no similar prohibition for diabetic drivers not using insulin. However, the FMCSA says these drivers could develop other disqualifying conditions if they don’t properly manage and control their diabetes, such as:
- Loss of limb or limb impairment;
- Cardiovascular issues;
- Loss of consciousness; or
- Other rheumatic, arthritic, orthopedic, muscular, neuromuscular, or vascular conditions.
The new form is open to public comment until late September. After that, the FMCSA will need approval from the White House before making the form available for use, a step expected to happen later this year.
Key to remember: Drivers who have diabetes but who don’t take insulin may soon be asked by their DOT medical examiner to provide them with a new assessment form. Once the form is available for use, drivers may want to be proactive and have their doctor complete the form before their next DOT exam.
