CSB investigates fatal incident involving liquid nitrogen
The U.S. Chemical Safety and Hazard Investigation Board (CSB) opened an investigation into a fatal incident involving liquid nitrogen at a poultry processing plant in Gainesville, Georgia. The investigators are interviewing key staff, examining perishable evidence, and connecting with local emergency management agencies. However, the main focus of the investigation centers around the plant’s cryogenic freezing system.
The incident
On January 28, a liquid nitrogen release at the plant resulted in six worker deaths, 12 worker hospitalizations, and the evacuation of the facility. Line workers use liquid nitrogen to flash freeze chicken. On the morning of the incident, the released liquid nitrogen rapidly converted to a gas that is heavier than air, which forced oxygen out of the room, effectively suffocating the workers.
The CSB investigation team will work with OSHA and other agencies to discover the cause of the event, including evaluating:
- Equipment and design;
- Regulation, industry standards, and guidance;
- Training, operations, and procedures; and
- Human and organizational factors.
The point of the investigation is to understand exactly what happened, what led up to the event, and why it occurred.
