Consider hazardous energy sources while troubleshooting equipment

Real event
What would you have done differently during this incident? During an industry incident, a blaster was having trouble with his electric 12-volt deadman line and was working on fixing the problem when his blast hose accidentally test-fired. The pot tender pushed the button on the solenoid of the 3.5 cubic feet abrasive blasting pot to reset the blast hose, resulting in the blast nozzle turning on.
The blast hose swung around and shot abrasive toward the right side of the worker’s face area. The pot tender who pushed the rest button assumed the blaster was still holding the blast nozzle. The blaster exited the platform he was working on and was immediately taken to an offsite medical facility, where he received first aid attention.
Contributing factors
Are your construction projects following proper safety requirements? The pot tender should have communicated with the blaster before pushing the reset button on the solenoid. Before servicing blast machines and equipment - or any tool or equipment - ensure that the electrical power is off, all air lines are shut off and bled, and the unit is stabilized. Lockout and/or tagout energy sources before working on the equipment.
Workers must follow the operating manual instructions to determine how to perform maintenance and servicing activities. Don’t make an assumption; when in doubt, contact a competent or qualified person and get guidance about how to do it safely.
Corrective actions
If this type of safety event happened at your jobsite, what would you do to ensure it didn’t happen again? All pressurized equipment must be bled off, and this must be verified as being complete by the blaster and the pot tender before performing any maintenance. LOTO should be used to ensure workers are isolated from hazardous energy sources.
Keys to remember
Workers must always report any defective equipment to their supervisor immediately. Any equipment that has a component or part that is defective or broken and could potentially present a hazard to workers must be taken offline and have an “out-of-service” tag attached in a conspicuous location. Always follow instructions in the owner’s manual for properly maintaining equipment at recommended intervals. Use LOTO to avoid exposure to hazardous energy sources.
