Colorado Workplace Public Health Rights Poster (PAID LEAVE , WHISTLEBLOWING , & PROTECTIVE EQUIPMENT)
Summary of state requirements: Workplace Public Health Rights: Under the Healthy Family and Workplaces Act, employers must provide each employee with a written notice containing information outlined in the Code of Colorado Regulations Rule 4.1.1 and may use the latest version of the Colorado Workplace Public Health Rights Poster to satisfy the notice requirement. The notice may be provided electronically or in hard copy and may be provided in a handbook or manual, or with other written or posted policies. A hard copy notice must be given to the employee. If the notice is provided in electronic form, the employee must easily be able to access the document electronically and be provided with notice that the document contains information regarding their terms of employment (not just a link that fails to notify the employee of this).

Notice of Income Tax Credits
Summary of state requirements: Employers must use Form DR 0995 to provide all employees who receive an annual wage and tax statement with written notice of the availability of the federal earned income tax credit, the state earned income tax credit, the federal child tax credit, and the state child tax credit. The notice must be provided at least once annually in English and in any other language the employer typically uses to communicate with employees. The written notice may be provided electronically via email or text message and may accompany an employe's wage and tax statement. Under Rule 39-22-604-4, an employer may provide the form or the entire written content of the form. If a printed form is provided, it must be no smaller than 5.5 x 8.5 inches, or half a standard sheet of paper. If it is provided electronially, the entire written content must be provided. (An employer does not meet the written notice requirement by providing an employee with only a link to the form.)

Employer/Employee Separation Form
Summary of state requirements: The employer must provide the employee with the Notice of Potential Availability of Unemployment Insurance Benefits form. The form must include:
- The employer’s name and address;
- The employee’s name and address;
- The employee’s identification number or the last four numbers of the employee’s Social Security number;
- The employee’s start date, date of last day worked, year-to-date earnings, and wages for the last week the employee worked; and
- The reason for separation.
