Owners and operators of UST systems must maintain a list of designated Class A, Class B, and Class C operators and maintain records verifying that training and retraining have been completed. The list must identify all Class A, Class B, and Class C operators currently designated for the facility and contain specific information about each operator, including:
- Name,
- Operator class,
- The date they assumed duties,
- The date they completed initial training, and
- The date (if applicable) they completed any retraining.
The records verifying the completion of training or retraining must:
- Be a paper or electronic record;
- At a minimum, identify the name of the trainee, the date they were trained, the operator training class completed, the name of the trainer or examiner, and the training company’s name, address, and telephone number; and
- Be kept for as long as the operators are designated.
Additionally:
- Records from classroom or field training programs (including Class C operator training provided by the Class A or Class B operator) or a comparable examination must, at a minimum, be signed by the trainer or examiner;
- Records from computer-based training must indicate the name of the training program and web address (if an internet-based program); and
- Records of retraining must include those areas in which the Class A or Class B operator has been retrained.
