Ohio has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to vehicles operated by for-hire carriers with a gross vehicle weight rating or gross combination weight rating, or gross vehicle weight or gross combination weight of 10,001 pounds or more (whichever is greater).
Section 390.15 applies to vehicles operated by private carriers with a gross combination weight rating or gross combination weight of 26,001 pounds or more, whichever is greater, inclusive of a towed unit(s) with a gross vehicle weight rating or gross vehicle weight of more than 10,000 pounds, or a single vehicle operated by a private carrier with a gross vehicle weight rating or gross vehicle weight of 26,001 pounds or more.
Section 390.15 also applies to any size vehicle (for-hire or private) transporting a placardable amount of hazardous materials.
Notification
An accident report is filed if one of the drivers involved in an accident is not covered by insurance. An injured party who cannot collect damages because the other driver did not have any insurance coverage may file an accident report within 6 months after the accident.

Procedure
A copy of an official traffic crash report investigated by the Ohio State Highway Patrol may be downloaded at: https://ohtrafficdata.dps.ohio.gov/crashordering/home.
Fee
A non-refundable fee of $4.00, payable to Ohio State Highway Patrol, is charged for each crash report copy. There is a $12.00 fee for photographs. All online requests must be charged to a credit card.
