Nebraska has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to all intrastate motor carriers and all vehicles of intrastate motor carriers with a gross vehicle weight rating, gross combination weight rating, gross vehicle weight, or gross combination weight over 10,000 pounds, and to all intrastate motor carriers operating vehicles transporting a placardable amount of hazardous materials.
Notification
The operator of any vehicle involved in an accident resulting in injuries or death to any person or damage to the property of any one person, including the operator, to an apparent extent of more than $1,500, must report the accident to the Department of Transportation within 10 days.
The accident report may be filed online or an accident report form may be downloaded, completed, and mailed to the Department of Transportation. Both online filing and the form to be downloaded and completed may be accessed at: https://dot.nebraska.gov/safety/crash-reporting/.
A report is not required to be filed if the accident is investigated by local law enforcement.
How to obtain accident reports
A copy of an official accident report may be obtained in person, by mail, or online. Each request must include the name of the driver involved and the date and location of the accident (including county).

Fee
A fee of $13.00 is charged for each copy of an accident report.
