Minnesota has incorporated by reference Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to motor vehicles that have a gross vehicle weight, gross vehicle weight rating, gross combination weight, or gross combination weight rating of 10,001 pounds or more, or hauling hazardous materials required to be placarded.
Notification
The driver of any vehicle involved in an accident resulting in injury or death of any person, or property damage to an apparent extent of $1,000 or more, must immediately notify local law enforcement.
How to obtain accident reports
Procedure
A copy of an official accident report may be obtained by sending a request in writing accompanied by the required fee to the Department of Public Safety. Such a request must be signed by an authorized requestor. The crash report request form is available at https://dps.mn.gov/divisions/dvs/Pages/default.aspx.
Fee
A fee of $5.00 is charged for each report request. Check or money order should be made payable to Driver and Vehicle Services (DVS).
