Iowa has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to motor vehicles that have a gross vehicle weight, gross vehicle weight rating, gross combination weight, or gross combination weight rating of 10,001 pounds or more, or hauling hazardous materials required to be placarded.
Section 390.15 does not apply to combination vehicles provided the gross vehicle weight rating of the power unit is 10,000 pounds or less and the gross combination weight rating is 26,000 pounds or less.
Notification
Motor vehicle accidents involving personal injury or death must be reported to the nearest office of the State Patrol or the nearest law enforcement officer. A written report must be made on any accident involving injury, death or combined property damage of $1,500 or more. The report must be submitted to the Department of Transportation within 72 hours of the accident. Failure to do so may result in suspension of driving privileges.
However, if the accident is investigated by a law enforcement agency, the driver is not required to file a report.
How to obtain accident reports
Procedure
A copy of an official officer’s accident report may be obtained only by any party to an accident, his/her insurance company or agent, or his/her attorney by sending a request in writing to the Office of Drivers Services, Department of Transportation. Request must include date of accident, time, location, and names of drivers and driver’s licenses of drivers involved. Request forms are available. A photocopy of the investigating officer’s report will be furnished.

Fee
$4.00 per copy (must accompany request).
