District of Columbia has adopted Sec. 390.15 of the Federal Regulations, which include accident recordkeeping requirements.
Applicability
Section 390.15 applies to vehicles with a gross vehicle weight rating or gross combination weight rating exceeding 10,000 pounds, and to any vehicle transporting hazardous materials requiring placards.
Notification
Any driver involved in an accident resulting in bodily injury (regardless of how slight) or death of any person or property damage amounting to $250 or more to the property of any one person must make a full report of the accident in writing on DPW 40-1 form to the Insurance Operations Branch, Department of Public Works within five days of the accident.
How to obtain accident reports
Procedure
A copy of an official accident report may be obtained by sending a request in writing to the Metropolitan Police Department. The following information must be submitted with the request: the complaint number (given to driver involved at the scene of the accident); the date and time of the accident; the location of the accident (street or avenue), and; the name(s) of the driver(s) or complainant.
Fee
A fee of $3.00 is charged for each report. Payment should be made by money order or check and payable to the “D.C. Treasurer”. A stamped, self-addressed envelope should be enclosed.
