Vermont has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to vehicles operating intrastate with a gross vehicle weight or gross combination weight rating of 26,001 or more pounds, or transporting hazardous materials requiring placarding.
Notification
Drivers of vehicles involved in accidents resulting in death, injury, or property damage of $3,000 or more must make a written report on forms provided by the state (TA-VA-04) to the Commissioner of Motor Vehicles within 72 hours of the accident.

Fees
$15.00 is charged for each copy of an individual accident report; $22.00 for a copy of the police accident report. Checks should be made payable to the Vermont Department of Motor Vehicles.
