Federal regulations
Utah has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Sec. 390.15 applies to an intrastate commercial vehicle:
- With a gross vehicle weight rating (GVWR), gross vehicle weight (GVW), gross combination weight rating (GCWR), or gross combination weight (GCW) of 26,001 or more pounds and is operated by an individual who is 18 years old or older;
- With a GVWR or GCWR of 16,001 or more pounds if the vehicle is operated by an individual under 18-years old; or
- Used to transport placardable hazardous materials.
Notification
A written report must be made within 10 days of an accident resulting in injury, death, or property damage amounting to $2,500.

An accident report may be accessed online at: https://highwaypatrol.utah.gov/online-accident-reports/.
Fee
- $5.00 for 1 to 10 pages
- $25.00 for 11 to 50 pages
