Yes. Employers can use an equivalent form, such as an incident investigation or workers’ compensation form, instead of the OSHA 301, as long as the form includes the same information, is easy to read and understand, and is completed following the same instructions as the OSHA 301. Many employers use their insurance carrier’s form for this purpose, or they use it and simply add any additional details OSHA requires. (29 CFR 1904.29)
