Kentucky uses federal OSHA injury and illness recordkeeping forms, and the state regulations do not include different requirements from the federal rule.

In the state of Kentucky, employers are required to report to the Division of Compliance 502-564-3070 (or OSHA after hours at 800-321-6742) any:

  • Work-related fatality with 8 hours. All fatalities must be reported.
  • The work-related in-patient hospitalization, amputation, or loss of an eye within 72 hours.
