Iowa has incorporated the federal OSHA recordkeeping regulations by reference, except reporting of fatalities and multiple hospitalization incidents.
Reporting work-related fatalities and hospitalizations (29 CFR 1904.39)
An Iowa employer must report the work-related death of an employee to Iowa OSHA within 8 hours. An employer must report to Iowa OSHA within 24 hours when an employee suffers one of the following as a result work:
Employers may complete an Incident Report Form online.
Employers may call 877-242-6742 or visit the Iowa OSHA office to report an incident.
