The General Instructions for Completing CSB Accidental Release Form relates to 40 CFR 1604, the Reporting of Accidental Releases Standard. Part 1604 requires an owner/operator of a stationary source to submit a report to the Chemical Safety and Hazard Investigation Board (CSB) for any accidental release resulting in a: (a) fatality, (b) injury or illness that results in a death or inpatient hospitalization, or (c) property damage equal to or greater than $1M. A release amount that results in any of those consequences would qualify for reporting.
An “accidental release” is an unanticipated emission of a regulated substance or other extremely hazardous substance (EHS) from the stationary source into the atmosphere inside or outside a stationary source. Regulated substances are those listed under 40 CFR 68, Risk Management Programs. The term EHS means any substance that may cause any of the three consequences.
If you have an accidental release, you are required to report it within eight hours of a qualifying event. One reporting method is to fill out and submit the CSB Accidental Release Reporting Form. The General Instructions for Completing CSB Accidental Release Form should assist you in filling out the form.
