Family and Medical Leave Insurance Eligibility Notice
Summary of state requirements: General notice must be provided to each employee by including the notice in employee handbooks or other written guidance to employees concerning employee benefits or leave rights, if these materials exist, or by distributing a copy of the general notice to each new employee upon hiring. This distribution may be accomplished electronically if all employees have access to a computer.

Summary of state requirements: Employers must provide written notices to each employee informing them of their rights and duties under this title both annually and at the time of hire. When an employee requests leave, the employer will notify the employee of their eligibility to take leave.
