Summary of state requirements: Employers that provide group health insurance coverage to employees must provide all eligible employees with a written list of the covered benefits included in the group health insurance coverage. This coverage must be provided annually, upon hire, and upon request in a format that easily compares those covered benefits with the essential health insurance benefits required of individual health insurance coverage regulated by the State of Illinois.

"What Every Worker Should Know about Unemployment Insurance" form
Summary of state requirements: When an employee quits or is discharged or laid off for an expected duration of seven or more days, the employer must provide the worker a copy of "What Every Worker Should Know About Unemployment Insurance" form in person upon separation. If delivery in person is impossible or impractical, the employer must mail the form to the worker's last known address within five days after the date of separation.
