Checking driving records
Information Required — Driver’s full name, date of birth, address and driver license number. The company’s name and address.
Format — The “Vermont DMV Record Request” form (VG-116) must be used. The form is available via the Department of Motor Vehicles website: https://dmv.vermont.gov/forms
Fee — $17.00 per request for certified copy of three-year operating record. $24.00 per request for certified copy of complete operating record. Fee must accompany request and be payable to VT Department of Motor Vehicles.
