Checking driving records
Information Required — Driver’s full name, date of birth, and driver license number. The company’s name and address.
Format — A “Request from Employer/Prospective Employer to Obtain a Complete Three-Year South Dakota Abstract of Driver’s Operating Record for Commercial Driver License Holder(s)” form must be completed and notarized. The form is available via the Department of Public Safety website: https://dps.sd.gov/records/driving-records.
Fee — $5.00 per request. Fee must accompany request and be payable to the Department of Public Safety.

Reading driving records
Driving record reports provided by the State of South Dakota indicate violations in full written out form using no coding.
