Checking driving records
Information Required —Driver’s full name and date of birth or driver license number. The company’s name and address.
Format — A “Records Request and Consent to Release” form (DPS303RM) is required.
Forms are available via the Department of Public Safety website: https://roar-assets-auto.rbl.ms/documents/15273/records_request.pdf.
Fee — $25.00 per uncertified record. $28.00 per certified record. A money order, cashier's check, personal check, or business check must accompany the request and be payable to the Department of Public Safety. Records may also be purchased at any Oklahoma Tag agency.

Reading driving records
Driving record reports supplied by the state of Oklahoma do not contain coding. All violations are written out in the report. Some reference state statute numbers.
