Checking driving records
Information Required — Driver’s full name, address, date of birth, and driver license number. The company’s name and address. The reason for requesting the abstract must also be provided.
Format — A “Request for Driver Abstract” form (SFN 51386) may be used to receive a complete copy of a driving record. A record may also be obtained online. The form and online request may be accessed at: https://www.dot.nd.gov/driver/driver-record-services-and-suspensions.
Fee — $3.00 per request. Fee must accompany request and be payable to the Driver’s License Division.

Reading driving records
A complete driving record, issued by the state of North Dakota, contains a driver’s total points and violations and convictions.
