Information Required — Driver’s full name, driver license number, address, and date of birth. The company’s name and address.

Format — All requests for Driver History abstracts must be submitted on Form DO-21. The form is available at: https://roar-assets-auto.rbl.ms/documents/16908/DO-21.pdf.

Reading driving records

Driving record reports issued by the State of New Jersey indicate violations in written out form along with codes, and on the reverse side is an explanation of some codes used on the report.
