Checking driving records
Information Required — Driver’s full name, address, date of birth and driver license number.
Format — Driving records containing personal information can only be requested by submitting Form 4681, “Request from Driver License Record Holder.” The form may be accessed via the Department of Revenue website: http://dor.mo.gov/forms/.
Fee — $2.82 per record.

Reading driving records
Driving record reports supplied by the State of Missouri indicate violations in written out form with few abbreviations.
