Checking driving records
Information Required — Driver’s full name, date of birth and driver license number. The company’s name and address, reason for request, and signature of requestor.
Format — Form DR-057 (individual record request) and Form DL-015 (multiple requests) may be used. A record may also be requested online. The forms and online requests may be accessed via the Motor Vehicle Administration website: https://mva.maryland.gov/Pages/driver-record-questions.aspx.
When listing more than five requests, the original and two copies must be submitted.
Requests must be typewritten on 81/2" × 11" white paper, no more than 22 requests per page, with each request double spaced. Return address and zip code must be on all copies of the list. Note: Only Maryland drivers licenses are to be listed.
Fee — $12.00 for a non-certified record, $15.00 for a certified record. Fee must accompany request and be payable to the Motor Vehicle Administration.
