Checking driving records
Information required — Driver’s full name, date of birth, and driver’s license number. The company’s name and address. A release must be obtained from the licensee before a traffic violation report can be given.
Format — Requests are to be filed on a “Driving Record Request” form and accompanied by an Arkansas “Driving Records Release” form signed by the driver. The forms may be accessed via the Department of Finance and Administration website: https://www.dfa.arkansas.gov/office/driver-services/driving-records/.
Fee — $8.50 for insurance record, $10.00 for job-related commercial (CDL) record.
