Checking driving records
Information required — Driver’s full name as it appears on the license, date of birth, and Arizona driver license number. The company’s name and address. The requestor’s signature must be notarized.
Format — Requests should be filed on Form 46-4416 “Motor Vehicle Record Request” form. The form may be accessed via the Motor Vehicle Division website: https://roar-assets-auto.rbl.ms/documents/14554/46-4416.pdf.
Fee — An uncertified 39-month record is $3.00 per request; any certified record (5-year or Driver History-Extended) is $5.00.
