Checking driving records
Information Required — Name, address, date of birth, Social Security Number, and driver license number.
Format — BMV 1173 “OBMV Record Request” form is required. If inquiring about another individual’s driving record, BMV 5008 “Notarized Written Consent Release of Personal Information” form must be submitted with BMV 1173. Forms are available via the Bureau of Motor Vehicles website: https://www.bmv.ohio.gov/doc-forms.aspx.
Fee — $5.00 per record.

Reading driving records
Driving record reports supplied by the State of Ohio indicate driver license status, accident involvement (fault/nonfault not indicated), traffic convictions, suspensions and revocations. Reports provide information for the preceding 3 years only.
