Ladderway floor holes or ladderway platform holes must be protected by a guardrail system and toeboards, except at the entrance of the ladder — where a self-closing gate or offset is required. Chains cannot be used.

Note that OSHA considers a fixed ladder access point to be a “hole” even when the ladder attaches to the edge of a platform or mezzanine.
