Fixed ladder platform requirements are outlined at 1910.28(b)(9)(D)(ii) which states that if a fixed ladder 24 feet or more above a lower level is equipped with a personal fall protection or ladder safety system, the ladder must have rest platforms every 150 feet or less.
If a fixed ladder 24 feet or more above a lower ladder has a cage or well (a cage or well is still allowed until November 18, 2036), then it must have landing platforms every 50 feet or less.
Employers may use a cage or well in combination with a personal fall arrest or ladder safety system provided that the cage or well does not inte3rfere with the operation of either system.
