Forms MCS-90 (for motor carriers) and MCS-82 (for buses) are completed by a carrier’s insurance agent when it registers the contractual agreement with the Federal Motor Carrier Safety Administration (FMCSA). Some carriers mistakenly believe that they must complete the form.
The insurance agent will send the carrier either an MCS-90 or MCS-82 as proof of insurance, and this form must be presented to the FMCSA in the event of an investigation or review.
