Endorsements for policies of insurance (Form MCS-90) and surety bonds (Form MCS-82) must be in the form prescribed by the Federal Motor Carrier Safety Administration (FMCSA) and approved by the Office of Management and Budget (OMB). Forms for passenger carriers (bus operators) use the same form number format, but the form number ends in a “B.” The agent completes and sends either form MCS-90 (insurance) or MCS-82 (bonds) to the carrier as proof of coverage. This form must be retained at the carrier’s office and presented as proof of financial responsibility in the event of an audit. Refer to 387.15.
