Federal regulations
Massachusetts has adopted Sec. 390.15 of the Federal Regulations, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to vehicles having a gross vehicle weight rating, gross vehicle weight, gross combination weight rating, or gross combination weight of 10,001 pounds or more that are used for the transportation of property, and vehicles transporting a placardable amount of hazardous materials.
Notification
Every person operating a motor vehicle which is involved in an accident in which any person is killed or injured or in which there is property damage in excess of $1,000 to any one vehicle or other property, must within five days after the accident, report in writing to the Motor Vehicle Registrar on an approved form. The form may be obtained via the Massachusetts Registry of Motor Vehicles website: https://www.mass.gov/doc/motor-vehicle-crash-operator-report/download
A copy must also be sent to the police department having jurisdiction over the place where the accident occurred.
How to obtain accident reports
Procedure
A copy of an accident report may be obtained by completing a “Request for Copy of Crash Report” form (21510). This form may be obtained via the Massachusetts Registry of Motor Vehicles website: https://www.mass.gov/doc/rmv-request-for-copy-of-crash-report-form/download.
