Kentucky has adopted Sec. 390.15, which includes accident recordkeeping requirements.
Applicability
Section 390.15 applies to for-hire and private vehicles with a gross vehicle weight rating, gross combination weight rating, gross vehicle weight, or gross combination weight of 10,001 pounds or more, or transporting hazardous materials which require placarding.
Notification
Any person operating a vehicle on the highways of this state who is involved in an accident resulting in fatal or non-fatal personal injury to any person or damage to the vehicle rendering the vehicle inoperable shall be required to immediately notify a law enforcement officer having jurisdiction. In the event the operator fails to notify or is incapable of notifying a law enforcement officer having jurisdiction, such responsibility shall rest with the owner of the vehicle or any occupant of the vehicle at the time of the accident. A law enforcement officer having jurisdiction shall investigate the accident and file a written report of the accident with his law enforcement agency.
Any person operating a vehicle on the highway of this state who is involved in an accident resulting in death, injury, or any property damage exceeding $500 in which an investigation is not conducted by a law enforcement officer shall file a written report of the accident with the Department of State Police in the Justice Cabinet within 10 days of occurrence of the accident upon forms provided by the Department.
How to obtain accident reports
Procedure
Those seeking copies of traffic accident reports investigated by the Kentucky State Police should contact in person or by written mailed request the appropriate Post according to the county of occurrence. The request should include the driver’s name, address, the county and city where the accident occurred, the highway number/name and investigating agency (if known), and the date of the accident. A written request should include a self addressed/stamped envelope.

Fee
$5.00 per report. Payment made by check or money order must be payable to the “Kentucky State Treasurer.”
