OSHA requires under 1910.23(d) that the side rails and grab bars of both through and side-step fixed ladders extend at least 42 inches above the top of the access level or landing platform served by the ladder. On December 17, 2019, OSHA issued corrections to its walking-working surfaces standard that included 42 inches is the minimum — not the exact — measurement for fixed ladder side rail extensions
