Checking driving records

Information Required — Driver’s full name, date of birth, and driver license number. The company’s name and address.

Format — “Privacy Act Agreement for Request of Motor Vehicle Records” (Form 431069) must be used. The form may be obtained via the Department of Transportation website: https://iowadot.gov/mvd/driverslicense/records.

Access to records is available online via the Department of Transportation website: https://driverrecords.iowa.gov/.

Fee — $5.50 per request. There is a $3.00 additional service fee to order online.
