Kentucky has adopted Part 391 of the Federal Motor Carrier Safety Regulations (FMCSRs) with modifications.
Part 391 applies to for-hire and private vehicles with a gross vehicle weight rating, gross combination weight rating, gross vehicle weight, or gross combination weight of 10,001 pounds or more, or transporting hazardous materials which require placarding

Driving Records | A specific form is not required. However, a request from an employer or prospective employer must include a signed affidavit from the driver in order to receive a full 5-year driving record. Driver records are also available online: https://secure.kentucky.gov/dhronline. The fee is:

Physical Exam - Motor Carrier | Physical exam and certificate required Though Kentucky has adopted the exception to the majority of the FMCSRs for transportation performed by the federal government, state government, or a political subdivision in 390.3(f)(2), drivers who possess a commercial driver’s license (CDL) must meet the medical examination requirements in Part 391 or possess a valid waiver. Medical waivers in addition to those allowed in 391.49 are allowed for drivers exclusively in intrastate commerce. Contact the Kentucky Transportation Cabinet for application procedures. |
