Arizona has adopted Part 391 of the Federal Motor Carrier Safety Regulations (FMCSRs), revised as of October 1, 2023 (and no later amendments or editions), with the exception of 391.11(b)(1).
Part 391 applies to a single motor vehicle or combination of motor vehicles which has a gross vehicle weight rating (GVWR) or gross weight of 26,001 pounds or more, and vehicles hauling hazardous materials in an amount required to be placarded.

| Driving Records | Requests should be filed on Form 46-4416 “Motor Vehicle Record Request” form. The form may be accessed via the Motor Vehicle Division website: https://roar-assets-auto.rbl.ms/documents/14554/46-4416.pdf. The fees are as follows: - $3.00 for an uncertified 39-month record
- $5.00 a certified:
- 5-year record or
- Driver-history extended record

| Physical Exam - Motor Carrier | Physical exam and certificate required. The Arizona Department of Transportation may grant an intrastate waiver under specified conditions to an individual: - Not meeting the vision requirements, or
- With a limb defect or impairment.
