Do we need a copy of the long physical examination form in our driver qualification files?
No, motor carriers are not required to have the examination form, only the medical certificates (wallet cards). The FMCSA does not prohibit carriers from requesting or obtaining the exam form, but it is not required. Carriers should have written driver consent to possess the long form.
