No, drivers are not required to have a copy of the long DOT physical examination form with them. A copy of the long physical form is not required to be in the driver’s qualification file either. The original of the physical form is retained at the medical examiner’s office. This is found in the instructions to the medical examiner in 391.43.
The driver is only responsible for carrying a legible copy of the medical examiner’s certificate on their person, per 391.41(a).
However, drivers who hold a commercial driver’s license (CDL) or commercial learner’s permit (CLP) who submitted their medical examiner’s certificate to the state licensing agency do not need to carry a copy of the certificate for more than 15 days after issuance. If the CDL or CLP driver’s motor vehicle record (MVR) doesn’t show the driver’s current medical certification information within 15 days after the medical exam it is prudent to carry the medical card until the MVR accurately reflects the current medical certifcation information.
