Question: Are report numbers required on the periodic (annual) inspection forms and decals typically used by a carrier? If so, who provides that number?
Answer: The report number is not a required entry on the inspection report. If entering one on the decal and report is preferred, the number is self-assigned by the company. Carriers with larger fleets use the report number to cross-reference a specific inspection decal to a specific inspection report, or to the maintenance record related to conducting the inspection.
