Wisconsin has adopted 390.15 of the Federal Motor Carrier Safety Regulations (FMCSRs). Section 390.15 includes accident recordkeeping requirements.

Section 390.15 applies to any self-propelled or towed motor vehicle used on a highway in intrastate commerce when the vehicle has a gross vehicle weight rating, gross combination weight rating, gross vehicle weight, or gross combination weight of 10,001 pounds or more, or is of any size transporting a placardable amount of hazardous materials.

Accident reporting - Motor carrier

NotificationMust report on state-issued form:
  • Death,
  • Injury,
  • Property damage ≥ $1,000,
  • Damage ≥ $200 to government property (except motor vehicles).
Reporting Timeframe 10 days (An accident report is not required if the accident is investigated by local law enforcement.)

Obtaining an accident report

ProcedureA copy of an official accident report may be obtained from the Wisconsin Department of Transportation, Crash Records Unit: https://crashreports.wi.gov/.
Fee$6.00 for each accident report
