The Washington State Patrol has adopted 390.15 of the Federal Motor Carrier Safety Regulations (FMCSRs), as it exists on April 25, 2025.
The Washington Utilities and Transportation Commission prescribes rules and regulations for household goods carriers, solid waste collection companies, and common carrier brokers in the state of Washington. The agency has adopted 390.15 of the FMCSRs, as in effect on December 31, 2024.
Section 390.15 includes accident recordkeeping requirements.
Section 390.15 applies to vehicles with a gross vehicle weight rating or gross combination weight rating or gross weight or gross combination weight of 10,001 or more pounds and vehicles transporting hazardous materials requiring placards.
Accident reporting - Motor carrier
| Notification | A report must be filed with the Chief of Police of the city or town if the accident occurred within an incorporated city or town or to the County Sheriff or State Patrol if the accident occurred outside a city or town, in triplicate on forms provided if the following occurred:

| Reporting Timeframe | 4 days (If police investigate and file a report on an accident, the drivers involved do not have to file a report.) |
Obtaining an accident report
| Procedure | A copy of a collision report may be obtained by completing a “Request for Copy of Collision Report” form. The form is available via the Washington State Patrol website: https://tinyurl.com/CrashWA1. A collision report may also be accessed via the Washington State Patrol website: https://fortress.wa.gov/wsp/wrecr/WSPCRS/. |
