Vermont has adopted 390.15 of the Federal Motor Carrier Safety Regulations (FMCSRs). Section 390.15 includes accident recordkeeping requirements.

Section 390.15 applies to vehicles operating intrastate with a gross vehicle weight or gross combination weight rating of 26,001 or more pounds, or transporting hazardous materials requiring placarding.

Accident reporting - Motor carrier

NotificationA written report must be made to the Commissioner of Motor Vehicles on a “Report of a Motor Vehicle Crash” form (VA-04) when the accident results in:
  • Death,
  • Injury, or
  • Property damage > $3,000.

Reporting TimeframeWithin 72 hours of the accident

Obtaining an accident report

Procedure The “Vermont DMV Record Request” form (VG-116) may be used.

Fee$15.00 for each copy of an individual accident report
$22.00 for copy of the police accident report
