New Jersey has adopted 390.15 of the Federal Motor Carrier Safety Regulations (FMCSRs), including all amendments. Section 390.15 includes accident recordkeeping requirements.

Section 390.15 applies to an intrastate vehicle with a gross vehicle weight, gross vehicle weight rating, gross combination weight, gross combination weight rating, or registered weight of 10,001 pounds or more, or a vehicle of any size transporting a placardable amount of hazardous materials.

Accident reporting - Motor carrier

NotificationAn accident must be reported to law enforcement and the Motor Vehicle Commission if the accident resulted in:
  • Death,
  • Injury, or
  • Property damage over $500.
A copy of the “New Jersey Self-Reporting Crash” form (SR-1), may be obtained at: https://www.state.nj.us/transportation/refdata/accident/selfreporting.shtm.
Reporting Timeframe
  • 10 days if report is not submitted by law enforcement officer.

ProcedureRequests for Non-Toll Crash Reports, New Jersey Turnpike Crash Reports, Garden State Parkway Crash Reports, and Records Requests for Serious/Fatal Crashes may be obtained at: https://www.njportal.com/NJSP/CrashReports/Home/Index.
Reports for accidents occurring on the Atlantic City Expressway are obtained from the specific roadway authority.

  • $13.00 for a report

Garden State Parkway and New Jersey Turnpike Authority:
  • $5.00 for a report
