Arizona has adopted 390.15, Federal Motor Carrier Safety Regulations (FMCSRs), revised as of October 1, 2023 (and no later amendments or editions). Section 390.15 includes accident recordkeeping requirements.

Section 390.15 applies to a single motor vehicle or combination of motor vehicles with a gross vehicle weight rating (GVWR) or gross weight of 26,001 pounds or more, and vehicles hauling hazardous materials in an amount required to be placarded.

Accident reporting - Motor carrier

Notification Report to local law enforcement if there is a:
  • Death
  • Injury or
  • Property damage over $2,000
Reporting Timeframe Immediately

Obtaining an accident report

Procedure Department of Public Safety investigated accident reports may be obtained online: https://www.azdps.gov/services/public/records/department.
Fee
  • $9.00 per accident report (first 9 pages, $1.00 extra for each additional page)
  • $4.00 per color photo
