Scenario 1 answer: Yes. A vehicle is considered out of service if it has even one of the defects or deficiencies listed in the North American Out-of-Service Criteria.

Scenario 2 answer: No. Windshield issues will not even result in a violation if there is any crack, discoloration or vision reducing materials, if:

1. The coloring or tinting applied at time of manufacture;
2. Any crack is not over 1⁄4 inch wide, and not intersected by any other crack;
3. Any damaged area not more than 3⁄4 of an inch (can be covered by a disc ¾ of an inch in diameter), if not closer than 3 inches to any other such damaged area;
4. The obstruction is a properly placed official label, sticker, decal, or part of a vehicle safety system.

Since the damage would not even qualify as a violation, the vehicle would not be placed out of service. (393.60)

Scenario 3 answer: Yes. A vehicle will be put out of service if any U-bolt(s), spring hanger(s), or other axle positioning part(s) in the suspension are cracked, broken, loose or missing resulting in shifting of an axle from its normal position. (393.207)
