1) Smoking in a non-smoking area. There is “No Smoking” sign in the background. Also, the cleaning solvent being used could be flammable.

2) Not wearing proper PPE. The cleaning solvent probably requires gloves at a minimum. The worker could also wear goggles and a splash apron.
