2) Poor housekeeping. A coil of wire is on the floor, and a piece of cardboard is on the pallet truck.

3) Poor location for tool box. Tool box is located under the mezzanine’s swinging gates with no toe board protection. If someone was working on the mezzanine, items could fall onto the person below.
